Introduction
Custom forms allow you to create tailored contact forms for your company website. This guide covers form creation, field customization, and multi-page setup.
Access Requirements
- Website administrator or owner permissions required
- Access through website > form builder
Creating a New Form
- Navigate to form builder
- Click "create a new form"
- Enter form name
- Save
Working with Fields
Field Types

Adding and Positioning Fields
- Add fields to the form by clicking and dragging on the six dots on the desired field.
- Reposition by clicking and dragging a field in the preview window.
Field Customization
- Click field to access editing options
- Options include:
- Required/optional status
- Input settings
- Label modification
- Names vs. Labels
- Label: Visible to form users
- Name: Shown to loan officers in leads
- Names and labels can be the same, but no two names can be the same.
- Each input requires unique name

- Note: Avoid changing predefined field input types
Removing Fields
Click the field in the preview window. Scroll to the bottom of the editor and click "Delete Field"
Multi-Page Forms

Each page requires a redirect button for form completion

Standard Method (Recommended)
Follow these steps for the simplest setup:
Important: Build complete form and name pages before configuring buttons to ensure proper redirection.

First Page

Click on the button field in the form preview window.

Change "Button Type" from save to redirect

Select correct page in "Redirect To" dropdown

Middle Pages

Use auto-generated buttons

Ensure redirect to next page using the same process as with the first page.

Page Before Thank You/Confirmation

Edit auto-generated button

Change "Next" to "Submit"

Set "Button Type" to "Save"

Note: For advanced users, custom button fields can be added manually, but this requires additional configuration and is more complex.
Form Settings
Number of Columns
- You can have 1 to 3 columns in your form. This changes it for all pages.
Notifications
Two types:
- Secure: Notification on submission only
- Show form data in email
Form Submission
- Submitted forms appear in Leads
- Loan officers receive notifications for forms filled out on their personal websites
